The City of London's one-page Zoning Compliance Letter Request Form, from Housing & Community Growth, Building Division. It collects the applicant's name, business name and contact details, the address of the request, and a detailed description of the requested use. A note at the foot gives the email address for the completed form and mentions the fee, paid by phone or email with a customer service representative. It belongs with the City's property information requests.
DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
