Overview

The City of Hamilton's request form for new addresses, unit assignments, or changes to existing municipal addresses and unit numbers, handled by the Planning and Economic Development Department's Legislative Approvals / Staging of Development group. It records the applicant's name and mailing address, the owner's contact information where the owner differs along with a letter of permission, the current address of the subject property, and the reason for the change or addition. It prints a $455.00 processing fee per property, and notes that departmental decisions on municipal numbers are final.

What it covers

  • Name and mailing address of the applicant
  • Owner (if different than applicant)
  • Current address of subject property
  • Reason for the requested change / addition to the existing municipal number(s) or unit
