The City of London's Heritage Alteration Permit Application Form, an 11-page package from Planning and Development. Five explanatory sections cover what a Heritage Alteration Permit is, when one applies under the Ontario Heritage Act, the application process, the information list and additional notes. Fillable Sections A to F then ask for the property and its designation, the proposed change, supporting material, applicant details and a sworn declaration, followed by an office-use block. It is the City's form for heritage designated properties and districts.
DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
