Overview

A one-page consent published by Urban Forestry in the City of Toronto's Parks, Forestry & Recreation division, used where a tree sits on private property on the border of the road allowance. The registered owner prints their name and address, requests City maintenance of the described trees, and signs an indemnity covering claims arising from the City performing or not performing that work, together with a three-year undertaking on pruning and removal. A City use only block records tree identifier, species, diameter, ownership and location detail.
