The City of Toronto's owner's authorization, filed where the applicant on the application for arborists retained by private property owner to undertake work on city trees is somebody other than the property owner. Two pages record the applicant's contact details, with a box for a single-name birth or change of name certificate, then the owners' signed authorization for a named agent, a corporate block for a signing officer binding the corporation, the agent's own signature, and a witness. A notice of collection closes it.
DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
