Overview

A single-page City of Toronto declaration in which the registered owners certify the use or uses proposed for the building named on a permit application. It carries a folder number and date, the project location by street number, street name and suite or unit number, a free-text block naming the declared use or uses, and signature, printed name and date lines for one or more registered owners. Its collection notice cites the City of Toronto Act and the Ontario Building Code Act.
