Mississauga's Form 2998, the Rental Repairs and Renovations Licence Application under Licensing By-law 0039-2026, filed with Licensing, Permits & Regulatory Services by a landlord who has served an N13 notice. Its two pages record a new or renewal licence and term, the residential complex, landlord and authorized representative, the rental unit's rent, size, bedrooms and tenancy, and the tenant. An Additional Requirements list names the N13 notice, building permit, tenancy agreement, fee, Qualified Person's report, $2,000,000 liability insurance certificate and signed release, followed by an acknowledgement and signature.
