The Rental Housing Demolition and Conversion Declaration of Use and Screening Form of the Town of Oakville, a seven-page form that helps determine whether By-Law 2023-102 applies to a proposal. After owner and project details it walks through Boxes 1 to 5 on dwelling units, rental units, condominium or life-lease status, changes to units and an application for consent, ending in a declaration and a definitions section. It is the form that comes before the Rental Housing Demolition & Conversion Application.
