Schedule 1 to an application for a permit to construct or demolish, filed once for each individual who reviews and takes responsibility for design activities, per Building Code Table 3.5.2.1 of Division C. It records the firm and individual, the classes of building and design activity involved — house, small, large or complex buildings; structural, HVAC, plumbing, on-site sewage systems and more — and the designer's declaration of registration or exemption under Division C, sections 3.2.4 or 3.2.5, with individual and firm BCINs.
