Schedule 2 to an application for a permit to construct or demolish, completed where the work involves an on-site sewage system. It asks whether the installer is engaged in the business of constructing, installing, repairing, servicing, cleaning or emptying sewage systems under Building Code Article 3.3.1.1 of Division C, and if so records the registered installer's name, BCIN and qualified supervisor, or notes the installer is unknown at the time of application, to be updated by a new Schedule 2 once known. The Town's second-suite step guide calls for it where a private septic system is involved.
