The City of Brampton's Schedule 2, used where a permit application involves an on-site sewage system. It asks whether the installer is engaged in the business of constructing, installing, repairing, servicing, cleaning or emptying sewage systems under Building Code article 3.3.1.1, Division C, with a third answer for an installer unknown at the time of application. Where the answer is yes, it records the registered installer's contact details and BCIN and the qualified supervisors, and the applicant's declaration closes the schedule.
