The City of Mississauga's Access Modification Permit Application (Form 2974) from Transportation & Works, for a curb cut, culvert change or new driveway, with its application checklist. The form takes site plan and building permit numbers, owner and agent details, property type, impacts on trees, above-ground utilities, culverts and road resurfacing, and supporting documents such as site plans, then general information on review time, fees and payment, and a curb cut and culvert diagram for detached, semi-detached and townhouse lots. The checklist adds technical criteria for drawings.
