The City of Mississauga's Certificate of Insurance for construction liability (Form 2591), from Legal Services, Risk Management, completed and signed by an insurance broker or insurer. It records the named insured, the work and the contract, file or permit number, and policies for commercial general or wrap-up, motor vehicle, umbrella and environmental impairment liability, with boxes for XCU, pollution, demolition, crane and blasting coverage. Its provisions add the City and the Region of Peel as additional insureds. Mississauga's road occupancy permit applications name it as their proof of insurance.
