Toronto Building's two-page appeal form, used after a sign variance application has been decided. It records the folder number, the variance decision number, and the street, lot and plan of the sign, then the appellant's contact details, a description of the variance that was applied for, and the reasons and justifications for the appeal. Its attachment list names a copy of the original decision and any supporting documents. An appellant's declaration on truth, copyright and accuracy closes it, and the collection notice cites Chapter 694, Signs, General.
