The City of Hamilton Building Division's one-page exterior waterproofing form, filled in with the property address and applicant contact details. It records the type of foundation wall — block, rubble or poured — with the form's own note about engineer-sealed plans where rubble is selected, which of the east, north, west and south walls are waterproofed, the length in linear feet, the materials used, whether weeping tile is installed or replaced, whether it ties into a combined sewer or a sump pump, and any decks or structures temporarily removed.

Check this against the municipality before you rely on it. DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.

Covers note
A single-page fill-in form: its content is a run of prompts and check boxes rather than titled sections, so there are no printed section headings to list.
