The City of Toronto Transportation Services application for a utility cut permit, filed on the utility cut permit path and emailed to utcuts@toronto.ca for short and emergency work or to fsutcuts@toronto.ca for full. Alongside the cut location, plant diagram, indemnity and lettered standard application conditions it shares with the non-utility cut form, it adds an application stream box for short, full or emergency work and a question on whether the work relates to a transit project confirmed by Metrolinx, the TTC or another transit group. Version stamp v: 2023.11.
