Appendix L to the City of Toronto's Municipal Consent Requirements for the Installation of Services within the City of Toronto Streets, version 4, carried on its utility cut permit page. The single page requests a new applicant code for the utility cut permit system: the company's corporate name, the forty character name associated with the code, the address to which permits and utility cut program mail go, an optional contact reference, a separate billing address, and a preferred three character code, with an office use block recording the code assigned.
DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
