Overview
The City of Kitchener's one-page 2018 Energy Efficiency Certification Form, issued under the Community Services Department letterhead. Boxes for the building envelope, mechanical systems, electrical systems and other disciplines hold a signature, date, name, address and professional seal, tied to Ontario Building Code Division B, Article 12.2.1.1. Check boxes list the ASHRAE and NECB energy efficiency levels and the exempt building types. It is one of Kitchener's commercial and industrial building permit forms.
