Form A1 of the Ministry of the Environment, Conservation and Parks, a one-page record for an alteration authorized for equipment that discharges a contaminant of concern to the atmosphere from a municipal sewage collection system, linked by the City of Kitchener on its sanitary and stormwater alterations page. Its four parts give the approval number, a description of the alteration, a verification by a Licensed Engineering Practitioner and a verification by the owner. It carries the note to retain the completed form.
