Form SW3 of the Ministry of the Environment, Conservation and Parks, a two-page record for an alteration authorized for third pipe collection systems, which handle foundation drainage and groundwater for non-potable reuse. The City of Kitchener links it on its sanitary and stormwater alterations page. Its five parts give the approval number, the description of the alteration, verifications by a Licensed Engineering Practitioner and by the owner, and a post-construction verification for inspection. It carries the note to retain the completed form.
DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
