Form SW1 of the Ministry of the Environment, Conservation and Parks, a two-page record for an alteration authorized for storm sewers, ditches or culverts, linked by the City of Kitchener on its sanitary and stormwater alterations page. Its five parts give the approval number, the description of the alteration, verifications by a Licensed Engineering Practitioner and by the owner, and a post-construction verification for inspection and testing. It refers to the Drainage Act and to Ontario Provincial Standards, and carries the note to retain the completed form.
