The City of Mississauga Building Division's one-page cancellation request, with tick boxes for an issued building permit, a request or application for a permit not yet issued, and a zoning certificate. It records the date, the requestor's name, company and phone number, the request, application or permit number, the address and the reason for cancellation, and is signed by both the requestor and the property owner.
