Commitment to Disconnect Services by Contractor/Owner

Work types

A one-page commitment the City of Hamilton publishes for buildings slated for partial demolition, completed by the owner or the owner's contractor. It records the project address, municipality, project description and permit application number, then carries two check-box warranties: one confirming that all services to the building have already been isolated or cut off from the portion slated for demolition, the other undertaking that they will be before any demolition works begin. It sits alongside Hamilton's separate Disconnection of Services form.

Check this against the municipality before you rely on it. DocHub collects what a municipality published; it cannot tell you what is in force today. A newer version may exist, may be titled differently, or may not be posted publicly at all.
